Eligibility

Individuals with a primary mental health need, including complex needs and dual diagnosis, who are at risk of losing their independence

Referral path

All referrals are via the East London Foundation Trust (ELFT) Luton Community Mental Health Team (CMHT). Referrals can also be made from other service professionals and local organisations seeking support for the people they work with. A panel assesses all referrals.
