A specialist outreach service working with people who are part of the street community or who are in supported accommodation and who have significant substance use issues.
Pathway’s Peer Mentor Service, in partnership with Medway Council Public Health, is a specialist outreach service working with people who are part of the street community or who are in supported accommodation and who have significant drug and alcohol problems. Individuals must either not be engaged with local treatment providers, or be registered but not engaging well with that support.
The project looks to engage people accessing the service in three main ways:
The service comprises an Outreach Coordinator, Outreach Worker, and Peer Mentors with lived experience who deliver assertive outreach by working closely with existing treatment service outreach teams. We work exclusively with people at risk of overdose who are either not under treatment services or not engaging with them.
Our team works across Medway’s supported housing provision and feeds into other projects. It aims to overcome barriers to engagement to ensure people access the proper support to reduce drug-related deaths and increase harm-reduction support for people with substance and alcohol use disorders, working with local treatment providers and other agencies.
People in Medway who have previously or are now at risk of experiencing rough sleeping, are at high risk of overdosing, or are either not under treatment services nor engaging with them.

Peer Mentors Needed:
If you’ve said yes to all of the above, there are people going through a similar experience that need your help in their recovery. You will be fully trained and supported by our Peer Outreach Coordinator throughout your time with the project.

The project will cover all expenses, including lunches, and you will be expected to offer 1-5 hours per week.
