How We Can Help:

STARS operates across Lewisham’s Complex Care Pathway to provide recovery-based and active rehabilitation to help reduce the step-up to Complex Care Services and improve the step-down from inpatient rehabilitation, residential care, and supported living into stable community accommodation.

 

Our dedicated team can provide peer mentoring, hospital in-reach and community-based support to people with severe mental health, significant comorbidities and complex social needs who need residential care, supported accommodation and inpatient rehabilitation.

What We Offer:

Our STARS Service offers a range of interventions:

 

  • Support with engagement and meaningful activities to help reduce boredom and promote social inclusion
  • Building relationships, structured routines and confidence to use public transport
  • Practical help and support with daily living and achieving personal goals
  • Support to attend appointments and fill out forms to access services and increase support
  • Support to access education and training opportunities

Eligibility

People with severe mental health, significant comorbidities and complex social needs who need residential care, supported accommodation and inpatient rehabilitation.

Referral path

All referrals are through the South London and Maudsley Complex Care Pathway.
