Eligibility

Adults with severe mental illness with a primary mental health need and dual health needs who require intensive and integrated rehabilitation in the community, including substance use, mild learning disability, challenging behaviour, physical disabilities, additional health needs and autistic spectrum conditions.

Referral path

Referrals can be made by a professional within a team responsible for a person’s care. Referrals will typically be inpatients in an acute ward or within an inpatient rehabilitation setting. Referrals are through SLP Complex Care Single Point of Access (SPA) at slm-tr.complexcarespa@nhs.net
