Read moreSLaM Community Rehabilitation Service is a mixed-gender community-based supported living service serving the South London and Maudsley (SLaM) NHS Foundation Trust boroughs of Croydon, Lewisham, and Southwark. We deliver specialist intensive rehabilitation to strengthen the level of care and support for people needing rehabilitation after discharge, helping them step down to less supported accommodation.
We can support adults with severe mental illness with a primary mental health need and dual health needs who require intensive and integrated rehabilitation in the community, including substance use, mild learning disability, challenging behaviour, physical disabilities, additional health needs and autistic spectrum conditions.
We deliver specialist intensive rehabilitation for up to nine months to support discharge from acute wards and avoid admission into inpatient rehabilitation. Our service operates across two sites and works closely with South London and Maudsley, which deliver clinical in-reach to deliver an integrated and rehabilitation-focused model of care.
Staffed 24 hours a day, we work intensively with service residents to identify and co-produce personal recovery goals, focusing on individual strengths and interests to meet their needs. Integral to our support programme is building:

Referrals can be made by a professional within a team responsible for a person’s care. Referrals will typically be inpatients in an acute ward or within an inpatient rehabilitation setting. Referrals are through SLP Complex Care Single Point of Access (SPA) at slm-tr.complexcarespa@nhs.net
