Service description
This is a care and support rehabilitation service for adults with complex mental health needs.
It provides 9-10 units of supported accommodation for individuals who may also have behaviours that prove challenging to services, including mental health medication compliance. It supports residents moving from a complex care inpatient rehabilitation service, Acute Mental Health Wards and (if appropriate a residential environment), initially into an intensive rehabilitation arrangement that works closely with the Mental Health Trust Community Rehabilitation.
A further 13-14 units provides move on supported accommodation from the intensive support service and provides an alternative to existing residential care provision. The transition allows them to further enhance their life skills and independence whilst still maintaining a tenancy in a 24-hour supported safe environment.
