We have a unique opportunity for a committed and experienced individual with professional expertise in mental health and well-being to join our CQC registered Care Home as a Health and Well-being Coordinator. Based in Bermondsey, South East London, Aspinden Care Home (ACH) provides accommodation for 26 people, male and female, who may have complex; chronic physical or mental health needs and/or those with long term alcohol addiction and require 24/7 care and support.
The successful candidate will be responsible for coordinating the day-to-day activities undertaken by the Personal Health and Well Being Practitioners in order to deliver a responsive and enabling service for people with complex needs in line with agreed personal care plans. Working collaboratively, you will offer practical-based support for people with substance misuse issues and mental ill health and actively help promote independence, dignity and quality of life at all times within a supportive environment.
Key Responsibilities:
- Assess referrals; provide key work, risk assessment and support planning for service users.
- Support and encourage service users to make choices about their care and support and ensure that these are communicated to those involved in the care of the service users.
- Take the lead and respond to any safeguarding concerns identified in conjunction with the service manager and their deputy.
- Coordinate and, where necessary escort, service user to appointments such as dentist, Court cases, DSS, and interviews for move on accommodation.
- Perform and/or assist services users to perform basic personal care tasks as outlined in the service users personal care plan, ensuring that they are supported to self direct the care and support offered.
- Undertake supervision for Personal Health and Wellbeing Practitioners.
Experience Required:
- QCF qualification or equivalent
- Proven experience of working in residential care setting.
- Good understanding of the effects of alcohol and the interventions and outcomes required by the service users.
- Experience of providing a harm reduction model of care.
- Knowledge of CQC and QAF outcomes framework
- Experience of administering Medication and support service users to move towards self medication.
- Excellent report writing, literacy and ICT skills.
- Experience of recovery focused service provision both in substance misuse and mental health.
